INSURANCE

Payment options include private payment or insurance reimbursement, depending on your insurance carrier and plan. Kristen is contracted and in-network with the following insurance companies:

• WELLMARK BCBS

• AVERA HEALTH PLANS

• SANFORD HELATH PLANS

If using insurance, your contracted rate for services is set by your insurance company. Please inquire with your health insurance provider regarding your mental health coverage, as your costs will depend on your specific plan. Each insurance company has a different reimbursement rate and contract to which your fees will adjust. You will receive a monthly invoice for co-insurance, copays, or deductible fees.

PRIVATE PAY

If I do not accept your insurance or you choose to pay privately, I have a sliding fee scale that sets your rate per session. Please reach out with any questions about my billing rates, sliding-fee rates, and payment plans. You will receive a monthly invoice for session fees.

OUT-OF-NETWORK INSURANCE

For out-of-network insurance billing, you may receive partial or full reimbursement by providing a superbill to your insurance company. Please inquire for more information regarding out-of-network insurance billing.
